BEFORE WE DIG IN
How Fifty 410 works + is it right for you?
Before reviewing, I signed up with Fifty 410, recorded the process, and gathered the key info you need to see if it’s a good fit. This review follows NNC’s Price & Trust Transparency Score methodology, which evaluates providers across true cost analysis and safety & legitimacy.
Disclosure: I’m a paid contributor to NutritionNC.com and was not compensated by Fifty 410 for this review.
Step 1: Sign up and complete your intake.
The intake was an average length and even quicker than some. It asked the standard personal and health questions and had me sign a couple of long agreements. A photo of a state-issued ID was required. No payment information was collected before the consultation, which is unusual.
Step 2: A provider reviews your file asynchronously.
Approval took less than 24 hours for me. There is no video visit. The consultation arrived in the patient portal, where I could see the provider’s name and credentials but not a license number.
Step 3: Choose a pharmacy and pay.
The consultation included a list of pharmacies with the price of the medication and a link to select and pay for the option I preferred. Payment takes place here, after the provider review.
Step 4: Receive your medication and start treatment.
My package arrived 26 days after approval in foam-insulated packaging with a gel ice pack, all injection supplies, and printed instructions inside.
Step 5: Complete monthly refills.
Refills run through an asynchronous monthly review. A refill questionnaire is sent to the patient portal each month, and a provider signs off without a live visit.
Fifty 410 carries only compounded semaglutide and tirzepatide injections. It also sells non-GLP-1 products, such as hair-loss treatments and longevity supplements like NAD+ injections and sermorelin.
Insurance Eligibility
Not accepted
Fifty 410 is a self-pay clinic and does not bill insurance for GLP-1 medications. It does offer payment plans through Afterpay and Affirm.
HSA/FSA Eligibility
Accepted
Fifty 410 accepts HSA and FSA cards for its medications.
Available States
Most states
Fifty 410 does not publish a list of the states it serves or excludes. It says it covers most US states, which depends on where its providers are licensed and where its pharmacy partners ship. I reached out, though, and was told Fifty 410 does not serve Alabama, Alaska, Mississippi, or South Carolina.
Customer Support
Limited
Fifty 410 offers email support only, at help@fifty410.com, with no phone number or live chat. Listed hours are Monday through Friday 9 a.m. to 6 p.m. CST and Saturday 10 a.m. to 4 p.m. CST, with a stated response time within 24 hours.
Coaching & Lifestyle
Not offered
Fifty 410 does not provide behavior coaching, nutrition support, or fitness guidance. It does host a patient community.
1 The safety and efficacy of compounded, oral, and microdosed medications, including treatments that have not been inspected and approved by the FDA, cannot be guaranteed. GLP-1 medications may not be appropriate for everyone. Patients should disclose a history of pancreatitis, gallbladder disease, severe gastrointestinal disease, kidney disease or dehydration risk, use of diabetes medications such as insulin or sulfonylureas, pregnancy or plans for pregnancy, any personal or family history of medullary thyroid carcinoma or MEN2, and a history of diabetic retinopathy. Patients should seek urgent medical help for severe or persistent vomiting, signs of dehydration, severe abdominal pain, especially radiating to the back, right upper abdominal pain, fever, jaundice, dark urine, pale stools, symptoms of severe hypoglycemia, allergic reaction, or possible overdose. Patients scheduled for procedures requiring sedation or anesthesia should inform their provider of GLP-1 use because these medications may slow gastric emptying and increase the risk of aspiration during peri-procedural periods.
Fifty 410 keeps pricing refreshingly simple. Your payment covers the medication, supplies, shipping, provider support, and access to its patient community, with no monthly membership layered on top. My true monthly cost landed at $199 for compounded semaglutide, and the site lists that number openly instead of burying it.
The one caveat is that the BBB has flagged complaints about advertised prices not matching what some customers were charged.
KEY OBSERVATIONS
Fifty 410 lists an all-in price for each medication, though tirzepatide does come with dose-based prices, and there is no separate subscription stacked on top. What you see is the full cost.
The price quoted in the consultation matched what I paid at the payment link. I did not run into hidden fees at checkout.
Fifty 410 does not currently appear to advertise specific pricing in its ads, though the BBB has noted past promotions like “starting at $199/month” that drew pricing complaints.
Pricing Breakdown
Initial Lab Test
Not included
Lab work is not included and not required. Fifty 410 does not ask for blood work before prescribing.
Monthly Subscription
None
Fifty 410 does not run a subscription. Each order is a one-time purchase, so there is no recurring membership fee to track or cancel.
Monthly Medication Costs
Compounded semaglutide (injection)
$199 3-month bundle: $299 (~$99/month)
Compounded tirzepatide (injection)
$299 Bundle pricing: 3 months for $399–$549; 6 months for $799–$1,099)
Bundles are paid in full up front. Fifty 410 uses dose-based pricing for its tirzepatide bundles.
ALSO INCLUDED
Coaching and lifestyle support Fifty 410 does not include behavior coaching or any structured lifestyle program. There is a patient community, but no guided coaching.
Nutritional support Fifty 410 does not provide nutrition coaching, meal plans, or dietary guidance with any order.
Doctor consultations An asynchronous provider consultation is included in the price. There is no live video visit, and follow-up reviews happen monthly through the portal.
Shipping Shipping is included at no extra cost and arrives overnight once the order leaves the pharmacy. The catch is how long it took to get there, which I cover below.
Safety & Legitimacy
Is this provider trustworthy and properly licensed?
Fifty 410 is LegitScript certified and operates from a verifiable US address, which are real positives. The concerns are transparency and pharmacy handling. Fifty 410 does not publicly name its pharmacies, but two of the three partners it named upon my request carry FDA enforcement records, and my own order shipped from a pharmacy that wasn’t even on the list I chose from.
KEY OBSERVATIONS
Fifty 410 holds active LegitScript certification, the industry gold standard that confirms basic safety and compliance standards.
Of the three pharmacy partners named on request, two (Apothecary and Hallandale) carry FDA enforcement records, but I found no active disciplinary records for OptioRx.
The listed business address checks out, though the BBB has noted there is no active Texas business filing under the company’s names.
Credentials Breakdown
LegitScript Certification Verified
LegitScript certification is the industry gold standard for online healthcare providers, and Fifty 410’s certification shows it meets rigorous standards for safety, compliance, and trustworthy online practices.
HIPAA Compliant Minor Issues
Fifty 410 references a HIPAA notice of privacy practices in its terms, but there is no direct link to it on the site. To get a copy, you would have to email help@fifty410.com. There is a privacy policy link in the footer, but the page is blank, which is a red flag whether it is intentional or just sloppy.
Clean Provider Records Major issues
Fifty 410 is a telehealth platform rather than a prescriber, and it connects you to state-licensed providers whose names you only see after intake. It does not publicly list its pharmacies either, naming them only on request: Apothecary/NewLife (503B), Hallandale (503B), and OptioRx (503A). The 503B partner behind the NewLife channel received an FDA Form 483 and a warning letter in 2025 for insanitary aseptic conditions, and Hallandale has a long disciplinary trail that includes an FDA warning letter and multiple state board actions. OptioRx had no enforcement records I could find. The bigger issue for me was personal: I selected one pharmacy at the payment link, but my medication arrived from Quaker Community Pharmacy, a name that wasn’t on the list and wasn’t disclosed to me before it shipped.
US Address Perfect
Fifty 410’s listed address is 1630 W Prosper Trail, Prosper, TX 75078, and it is verifiable through FDA records, USPTO trademark filings, and the company’s own terms, which list the legal entity as Aspen Aesthetics dba Fifty 410. However, the BBB says it could not find an active Texas business filing under those names, and in February 2026, the FDA sent Fifty 410 a warning letter about misleading website labeling and “clinically proven” language. The letter concerned marketing rather than drug safety, but it is still a regulatory mark against the company.
Patient Support & Comprehensive Care
How easy is it to get help from real people?
Getting help from Fifty 410 means email, and only email. There is no phone number or live chat anywhere on the site, which is unacceptable for a company handling prescription medication. Clinical oversight exists, but the details were hard to find, and the patient portal is the most bare-bones I have seen.
KEY OBSERVATIONS
The portal has a single message section where every reply piles into one long thread, which would be a chore to scroll for past information.
Fifty 410 does not publish a phone number on its contact page, homepage, or footer. Email is the only listed channel.
My email got a real human reply within a couple of hours. It was brisk and short, but it directly answered my question.
Support & Care Breakdown
Live Chat Not offered
Fifty 410 does not offer live chat. The patient portal has a single message thread where all messages stack into one continuous conversation, which makes finding past information annoying.
Phone Support Not offered
Fifty 410 does not publish a phone number anywhere on its website. There is no way to call the company.
Email Human
Email at help@fifty410.com is the only real way to reach Fifty 410. I received a genuine human reply within a couple of hours that answered my question directly. It was short and to the point—not rude, just very brief.
Clinical Access & Oversight Okay
Clinical access runs through the patient portal chat, and you see a provider through an asynchronous monthly consultation by completing a refill questionnaire. This was not clearly explained anywhere, and I had to reach out to learn the refill link is sent to the portal each month.
Medication Handling & Shipping Standards
How well shipped and packaged are the medications?
Fifty 410 handled the cold chain and supplies well, but the delivery process dragged badly. The medication arrived in solid insulated packaging with an ice pack and a complete set of supplies. The problem was it took 26 days to ship after approval, and tracking did not show up until the day it arrived—more like a delivery confirmation than tracking at all.
KEY OBSERVATIONS
The medication shipped inside a foam-insulated package with a single gel ice pack. Temperature control was handled properly.
A separate ziplock bag held the injection supplies, medication information, and instructions, with extra sharps and alcohol wipes and no branded materials.
Tracking was sent by text message the same day the package arrived, with no update in the patient portal at any point.
Shipping Standards Breakdown
Cold-Chain Shipping Standards Perfect
The medication arrived inside a foam-insulated package with a single gel ice pack. Temperature control was done right, with no concerns about how cold the package stayed.
Packaging Quality & Security Minor issues
The medication came as a vial packed inside a very large pill bottle, so the vial had a lot of room to shake around in transit. That said, the contents showed no bubbles or visible damage when I inspected them.
Supplies & Instructions Perfect
A separate ziplock pack held all the injection supplies, the medication information, and clear instructions, plus extra sharps and alcohol wipes. There were no branded materials. The medication came as a vial, so you draw and inject yourself, and the instructions covered storage, temperature handling, and how to inject.
Tracking & Delivery Major issues
This was the weakest part of the experience. After approval came in under 24 hours, it took 26 days for the pharmacy to actually ship my order, even though the site advertises one- to two-day delivery. The shipping leg itself was fast (overnight once it left), but no shipping information was provided during the long wait, and tracking only arrived by text message on the day the package showed up, with nothing in the portal.
Transparent Operations
How honest is Fifty 410 in its policies and communications?
Fifty 410 scored near the top here, and most of it is earned. Pricing is clear, there are no pressure tactics, patient images look authentic, and the company makes no unverifiable media claims. The small deductions come from the cancellation and refund window and the fact that it carries only compounded medications.
KEY OBSERVATIONS
Because each order is a one-time purchase, there is nothing recurring to cancel, and refunds are available before compounding begins.
Fifty 410 does not plaster its site with media logos or unverifiable “as seen on” claims.
I did not run into countdown timers, urgency messaging, or forced upsell prompts anywhere in the process.
Operations Breakdown
Cancellation and Post-Cancellation Billing Minor issues
Because Fifty 410 does not require a subscription, there is nothing recurring to cancel. Each order is a one-time purchase. To cancel an active order, you email help@fifty410.com or use the request-a-refund button on the refund policy page. The timing matters; you get a full refund only if you cancel before the pharmacy begins compounding your medication, and once it ships there are no refunds. I did appreciate that Fifty 410 does not store payment information.
Clear Pricing and Policies Perfect
Pricing is listed clearly as an all-in fee. You can get a full refund anytime before compounding starts, and after shipment only replacements for incorrect orders or refunds if you are medically disqualified before shipment.
Accuracy and Authenticity of Images Perfect
The patient and outcome images on Fifty 410’s site appear authentic, with no signs of AI-generated photos.
Advertised Pricing Accuracy Perfect
The price quoted to me matched what I paid at checkout, with no hidden fees. Worth noting, though, is that the BBB has flagged complaints alleging that advertised prices did not match final charges, and it asked Fifty 410 for enhanced pricing disclosures. Pricing should be disclosed fully and without surprises.
Media and Third-Party Validation Perfect
Fifty 410 does not make media, award, or partnership claims, and I saw no media logos on the site.
Marketing Practices and Sales Pressure Perfect
I did not experience any aggressive sales tactics. There were no urgency prompts or forced steps pushing me toward a purchase.
Clarity Around Medication Types and Limitations Minor issues
Fifty 410 carries only compounded GLP-1 injections, which lack the established safety and efficacy data of branded products. To its credit, the site offers extensive information explaining how compounded versions differ from the name-brand drugs.
Patient Reviews & Real Experiences
What do recent customers think?
Fifty 410’s customer sentiment is genuinely strong. It holds a 4.8 on Trustpilot across nearly 5,000 reviews, and Reddit users generally praise its responsive service and fast shipping. But the BBB, where it carries an F rating for unanswered complaints and minimal review engagement, tells a different tale. On balance, the public feedback leans positive.
KEY OBSERVATIONS
Fifty 410 holds a 4.8 rating on Trustpilot across roughly 4,992 reviews, which is excellent for the category.
The BBB gives Fifty 410 an F rating for failing to respond to complaints, and it has open questions about the company’s business filing and pricing.
Reddit users describe Fifty 410 as responsive with fast shipping, while noting it is not always the cheapest option.
Who Should Use Fifty 410?
If you want all-in pricing with no subscription to manage, then Fifty 410 fits well. Each order is a one-time purchase that covers the medication, supplies, shipping, and provider support. No membership fee is stacked on top of your medication costs.
If you prefer paying after a provider has reviewed your file, then Fifty 410’s flow suits you. No payment information is collected during intake, and you only pay once the consultation is over and medication options come back.
If a LegitScript-certified clinic with strong customer sentiment appeals to you, then Fifty 410 is worth a look. It holds active certification and a 4.8 Trustpilot rating across nearly 5,000 reviews.
If you want to commit to a longer supply for a lower per-month cost, then Fifty 410’s bundles deliver. A three-month semaglutide supply drops the effective cost to about $99/month.
Who is Fifty 410 Not For?
If you need your medication quickly, then Fifty 410 is a risk. My order took 26 days to ship after approval, despite the site advertising one- to two-day delivery.
If you want to reach a human by phone or chat, then Fifty 410 will frustrate you. Email is the only support channel, with no phone number or live chat anywhere on the site.
If knowing exactly which pharmacy fills your prescription matters to you, then Fifty 410 falls short. It does not name its pharmacies publicly, and my order shipped from a pharmacy I never selected and was never told about.
If you want branded GLP-1s like Ozempic, Mounjaro, Wegovy, or Zepbound, look elsewhere.Mochi Health offers a much wider selection of both branded and compounded options.
If you want one predictable price for tirzepatide, then Fifty 410’s dose-based pricing may frustrate you. Its tirzepatide bundles are priced by dose, so the cost rises as your dose increases.
Final Verdict
OVERALL SCORE
Good
Fifty 410 scores 80/100, and its biggest strength is transparent pricing with no subscription to manage.
The standout positives are honest, all-in pricing and strong public sentiment, including a 4.8 Trustpilot rating. The standout weaknesses are a 26-day wait before my order shipped, dose-based pricing for tirzepatide, and support that is limited to email, with no phone or chat. There were also pharmacy transparency gaps I could not get comfortable with.
If you value simple pricing and don’t mind waiting, Fifty 410 can work, but if you need fast delivery or real-time support, look elsewhere.
Take the first step toward better health. Get evaluated now.
Fifty 410 charges $199/month for compounded semaglutide injections and $299/month for compounded tirzepatide injections. There is no membership fee, and three-month and six-month bundles lower the per-month cost. Semaglutide drops to about $99/month on the three-month supply. Bundles for tirzepatide operate on a dose-based pricing system but still offer considerable monthly savings.
Does Fifty 410 accept insurance or HSA/FSA?
Fifty 410 does not accept insurance and is self-pay only. It does accept HSA and FSA cards, and it offers payment plans through Afterpay and Affirm.
Is Fifty 410 a legitimate GLP-1 provider?
Fifty 410 is LegitScript certified and operates from a verifiable US address as Aspen Aesthetics dba Fifty 410. That said, it does not publicly name its pharmacies, and it received an FDA warning letter in February 2026 over misleading website labeling.
How long does Fifty 410 take to ship?
Fifty 410 advertises one- to two-day delivery, but my order took 26 days to ship after approval. The shipping leg itself was overnight, but the pharmacy fulfillment delay was the slowest I have tested.
What medications does Fifty 410 offer?
Fifty 410 offers compounded semaglutide and tirzepatide injections only. There are no branded GLP-1s (Ozempic®, Mounjaro®, Wegovy®, Zepbound®), no oral formats, and no microdose options. It also sells non-GLP-1 products like hair-loss treatments and NAD+ injections.
