GLP-1s are an incredible advancement in treating obesity, but there’s more to prolonged weight loss than injecting ourselves. Most of us need to also make lifestyle changes, as well as getting reliable medical guidance when it comes to managing our GLP-1 use.
That’s why the clinic that prescribes your weight-loss meds can be as important as the meds themselves.
Out of all the online weight-loss clinics out there, Form Health stands out for two reasons: its quality lifestyle coaching and its steep price.
📋Our Score: 4/5
High costs and some lack of transparency prevent Form Health from being a truly elite clinic, but if you’ve got a good insurance policy, you might have found the perfect clinic for you.

What Is Form Health?

Form Health is a virtual weight‑loss clinic that pairs you with a board‑certified obesity doctor and registered dietitian to accompany your GLP-1 prescription.
Using the Form Health app, you’ll work one-on-one with an obesity medicine physician and registered dietitian to create a comprehensive plan that can include FDA-approved GLP‑1 medications like Wegovy®, Zepbound®, and Mounjaro®.
Unlike the many online clinics that only offer scripts, Form Health emphasizes coaching, behavior, and sustainable lifestyle changes.
Form Health’s goal is long-term weight loss that lasts well beyond your GLP-1 prescription.
How Does Form Health Work?

Form Health follows a pretty similar format to most other telehealth weight-loss clinics, but its payment screen may come as an early surprise.
Step 1: Three-Minute Quiz
Form asks you some quick, simple questions about your motivation for weight loss, as well as pre-existing conditions, where you live, which insurance you have (if any), and your current weight and height to calculate your BMI.
Step 2: Payment
A lot of weight-loss clinics expect you to sign up before you receive your prescription or meet with a clinician, but it seems like Form gets to the payment screen in record time. It’d be nice if it broke the mold, but your first monthly payment is due immediately.
Step 3: Download the Form Health App
The hub through which you’ll do almost all your interactions is the Form Health App. You’ll use it to make your appointments, track your results, get messages from your dietician and the care team, and more.
Step 4: Formulate Your Treatment Plan
Through the aforementioned app, you’ll have appointments with your doctor and dietician, and together you will come up with a plan.
If your doctor believes GLP-1s are right for you, they will issue you prescriptions at this time for FDA-approved weight-loss meds like Wegovy or Zepbound.
Step 5: Ongoing Care
Form Health really excels at ongoing care. The care team reaches out to you regularly, and you’ll also receive encouragement and further guidance from both your doctor and your dietician.
You can even send your care team meal photos for critique and encouragement.
Should you need to make any adjustments in your program, Form Health will be quick to respond and come up with alternatives.
Treatment Options at Form Health Review

Form Health only offers FDA-approved medications, including three GLP-1s, in addition to lifestyle coaching and help from certified dieticians.
Tirzepatide (Zepbound)
Form Health offers Zepbound in preloaded, self-injecting pens that you use once a week. These pens provide a convenient form of injection so you don’t have to mess around with syringes.
Semaglutide (Wegovy)
Wegovy is also offered by Form, and, like Zepbound, comes in the form of a preloaded pen and is self-injected once a week.
Liraglutide (Saxenda®)
Saxenda also comes as preloaded injectable pens. The big difference between Wegovy and Saxenda is that Saxenda must be injected once a day. That’s obviously a much larger time commitment and more of an inconvenience, but some may prefer the consistency of a daily routine.
Contrave®
Contrave comes in tablet form, so it’s great for those who hate injections, and has two active ingredients: bupropion and naltrexone. They are supposed to help curb food cravings, but Contrave has not proven as effective as GLP-1s.[1]
According to Form, 5.4% of Contrave users lost more weight while increasing physical activity and following a reduced-calorie diet, much more than the 1.3% who lost weight following the same regime without Contrave.
By contrast, 36% of Zepbound users (15mg) lost more than 25% of their body weight in 72 weeks, according to Eli Lilly.[2]
Lifestyle coaching
Form Health really goes above and beyond most online weight-loss clinics when it comes to lifestyle coaching. You will speak to or chat with a registered dietician several times a month, support will check in with you if you haven’t reached out, and physicians are an ongoing part of your treatment, even months after you’ve begun a GLP-1 regimen.
Form Health Pricing: Pretty Pricey If You’re Not Insured
Form Health really wants to bill insurance companies rather than uninsured individuals. If you have insurance, its team will help navigate the sometimes tricky world of authorizations and reimbursements.
📝 Editor’s note
Prices are accurate at the time of publication. We update frequently, but pricing can change quickly.
According to Form, its doctor visits, lab work, and medication costs are covered by most major private insurances and Medicare. You would then only be responsible for any copay or deductible, just as you would for a normal, in-person doctor visit.
But if you don’t have insurance, you’ll pay $299/month before you even get to the costs of labs or medications.
- Covered with insurance (less copays and deductibles):
- Doctor visits
- Lab work
- Medication costs
- Without insurance:
- $299/month membership includes doctor and dietician appointments
- Lab work not included; price depends on lab and location, but average is $35–$150
- Medications not included (average $349–$1500+, depending on brand)
Unlike most online weight-loss clinics, Form doesn’t advertise how much your GLP-1 medications might cost. Instead it suggests you go directly to the “medication manufacturer site” for pricing information.
It doesn’t seem like Form Health is marking up medication prices, but it would be really nice to be 100% certain of the price of its meds before throwing down $299/month.

How it compares:
Almost every online weight-loss clinic is more transparent than Form Health. Here are some of their prices. I’ll use Zepbound (a semaglutide) for comparison since that is the only price Form Health reveals.
- Shed offers an optional $49.99/month coaching program that’s comparable to Form’s. In addition to Wegovy and Zepbound, Shed offers compounded semaglutide and compounded tirzepatide, which are not FDA approved but cost only $199+.
- PlushCare charges $19.99 a month for (very limited) support services, $15 for lab work and $119 for doctor’s appointments. Its price for Zepbound is $349–$449/month.
- WeightWatchers charges $74/month for ongoing care, although its customer service receives a lot of complaints and isn’t nearly as good as Form’s. It charges $349+ for Zepbound.
In general, Form Health is expensive for those without insurance coverage. It charges more than 4x more per month for the membership than Shed does for its coaching program. Form Health’s ongoing care is definitely better, but probably not 4x better.
What’s Included in Form Health’s Membership Price?
If you don’t have adequate insurance, Form charges a membership price which covers everything but lab work and medication:
- Physician visits
- Dietician appointments
- Unlimited messaging to your care team
- Access to the Form app
- Access to private Facebook community
The physician and dietician visits are the best feature of the membership price and can add real value by making sure you get a truly personalized treatment plan.
What’s Not Included
With such a steep price tag, you might assume Form Health’s membership is all-inclusive, but it leaves out a couple of key elements:
- Lab costs
- Medication
Not all weight-loss clinics require lab work, but if you run into complications with your GLP-1 regimen or have other issues, you’ll need blood work done, which could run you $100 or more.
🎯 Who Should Use Form Health?
➡️ If your insurance policy covers the doctor appointments and medication, you should absolutely use Form Health.
Form Health is one of the absolute best weight-loss clinics when it comes to overall care and long-term weight management. If your insurance company will pick up all or most of the tab, you should definitely give it a shot.
➡️ If you need a lot of support beyond just a prescription and a “good luck!” you should go with Form.
Form will prescribe you GLP-1s, but it will also closely monitor your progress, encourage you along the way, reassess your prescription if things aren’t working, and look out for your long-term health beyond your prescription.
🚫 Who Is Form Health Not For?
If you don’t have insurance, Form Health is not the way to go. It charges a high monthly membership fee, and you still have to pay for lab work and meds.
To make things worse, it’s not very transparent about these costs. Form is one of the few telehealth clinics we’ve seen that doesn’t actually show pricing for all its GLP-1 options.
It also has a limited selection of GLP-1 options, offering no compounded medicines. Compounded medicines certainly have a level of risk that branded GLP-1s don’t, but if you find a trusted provider with an accredited pharmacy, compounded GLP-1s can be a much more affordable alternative.
Lastly, if you are in a hurry and just want a GLP-prescription without downloading apps, waiting long for an appointment and having to talk to a dietician, you can find faster, cheaper clinics that will quickly give you a prescription and send you on your way.

Real User Experience With Form Health
In general, Form Health has excellent customer feedback, and more than 84% of its clients leave five-star reviews on TrustPilot.
When people have logged complaints, which isn’t often, it’s usually because of insurance issues that force patients to pay out of pocket.
