Best Tirzepatide Compounding Pharmacies in 2026: Ranked & Reviewed

Updated on September 7, 2026

We ordered tirzepatide from nine telehealth companies, opened every box, and then looked up the pharmacy that filled each one. Three of those pharmacies have problems on record. One is closed. We paid for our own orders, contacted support on every channel, and canceled each account. Prices were re-checked on each company’s website on July 31, 2026.

How we tested this

We paid for our own orders. No free samples, no sponsored placements.

For each company we signed up, paid, received the medication, opened and photographed the packaging, contacted support through every channel they offered, and then canceled. We wrote down what happened at each step.

Then we found out which pharmacy filled the order. Sometimes it was listed on the website. Sometimes we had to ask support directly. Once we had a name, we searched state pharmacy board records for license status and disciplinary history.

Prices were re-checked on each company’s website on July 31, 2026.

Two things we did not do. We did not send the medication to a lab, so we cannot tell you whether any vial contains what the label says. And we cannot see inside every order: most of these companies use several pharmacies and route your prescription based on your state, so the pharmacy that filled our box may not be the one that fills yours.

Three companies touch your prescription. You see one.

When you order tirzepatide online, your money and your prescription pass through three separate businesses.

The website takes your order. A doctor reviews your file and writes the prescription, and in almost every case that doctor is contracted rather than employed. Then a compounding pharmacy mixes the drug, fills the vial, and ships the box.

You deal with the first one. The third one makes the thing you inject into your body.

That matters when something goes wrong. If a vial is the wrong strength or is not sterile, that is the pharmacy’s doing. Your state pharmacy board publishes license status and disciplinary history, and it is free to search. But you can only search it if you know the name.

503A and 503B, in plain terms

You will see these two labels on provider websites. Here is the difference.

A 503A pharmacy mixes medication for one patient at a time against one prescription. Your state licenses it. The FDA does not inspect it as a drug manufacturer. Most online GLP-1 prescriptions are filled this way.

A 503B facility is larger. It registers with the FDA, gets inspected by the FDA, and follows federal manufacturing rules. It can make batches without a prescription for each one.

Neither one makes an FDA-approved drug. The difference is the paper trail: a 503B has a federal inspection record, a 503A has a state one.

Watch for wording that borrows credibility without earning it. “FDA-registered labs” is not a 503B facility. “FDA-regulated facilities” is not either. Both phrases appear below.

Compounded tirzepatide is not FDA-approved. The FDA does not review it for safety, quality, or whether it works, and it is not the same product as Mounjaro or Zepbound.

Nobody publishes independent lab results showing their tirzepatide is the strength it claims. Not one of the nine.

1. Peak Wellness

Peak Wellness compounded tirzepatide

One named pharmacy, clean record. Score 87. $179 the first month, then $279.

Peak Wellness is the only company here whose pharmacy came back clean. It dispenses through a single named 503A partner, Emerald Compounding Pharmacy of Tampa, Florida (license PH35337), with no disciplinary actions on file.

It also publishes more than almost anyone. The site lists each pharmacy with a phone number and street address, and it names DrTelX as the group that handles consultations and prescriptions. Most companies will not tell you who writes your prescription at all.

Our box arrived with two small gel ice packs in a small insulated foam box. Small is better here, because there is less room for the vial to move. The medication was in a pill bottle inside a bubble wrap pouch, with two packs of sharps and alcohol wipes, plus a goody bag of ginger chews and mints for nausea. We have not seen that from anyone else.

Pricing is the easiest to understand on this page, and Peak Wellness is the only company that promises in writing not to charge more as your dose goes up. The site says “Flat Rate Pricing No Matter the Dose.” You pay $279 a month at 2.5 mg and $279 a month at 15 mg. The doctors are independent providers, not employees.

What we likedWhat we did not
No disciplinary action against its pharmacyAbout $3,248 a year, well above the cheapest options here
Names every pharmacy with address and phone, plus the prescriberCanceling means contacting support 5 days before your next shipment
Same price at every dose, promised in writingSite shows only the discounted first month, so real pricing appears after intake
Thoughtful packaging, extra supplies, nausea remediesSupport is human but limited for non-patients

Peak Wellness is not the cheapest option on this page and this article is not going to pretend it is. What it is, on the evidence we gathered, is the cleanest pharmacy record of the nine.

2. GobyMeds

GobyMeds compounded tirzepatide

Names the pharmacy during checkout. Score 89. From $133 a month.

GobyMeds does something no other company here does. It puts the pharmacy name and the additive right on the medication options while you are choosing, during intake. You know where your medication is coming from before you pay, not after you ask.

The pharmacies are CasaPharmaRx, SevenCells, VitalRx, and BPI Labs. CasaPharmaRx had nine compliance deficiencies found in August 2025 and corrected all of them by January 2026, with no prior disciplinary orders. The others came back clean.

The box was straightforward: one large ice pack in an insulated foam box, medication in a bubble wrap pouch inside a ziplock with the supplies, plus instructions and drug information.

Now the price, because the cheap number is a starter rate. A three-month starter bundle is $399, which is $133 a month. Plans up to 9 mg a week are $499. Plans at 11 mg a week or more are $599. That is roughly a 50% climb, and it happens right where most people end up. To its credit, GobyMeds says so on its own pricing page rather than hiding it.

What we likedWhat we did not

Cheapest full year of any company hereNobody answers the phone; chat replies took about 24 hours

3. Pomegranate

Pomegranate compounded tirzepatide

Five pharmacies, the clearest pricing we have seen. Score 94. $149 a month.

Pomegranate has the highest score in our data and the best pricing transparency of any company we have tested. There is a “Pricing” tab at the top of the site that opens an interactive tool showing cost per pharmacy and per dose. Nothing else in this market comes close.

The pharmacies are BPI Labs, Empower, Hallandale, RedRock, and OptioRx. Empower and Hallandale are among the larger compounders serving telehealth. We found no disciplinary history for Hallandale, BPI Labs, OptioRx, Empower, or most RedRock locations. One RedRock location in Springville has a paid $750 citation from 2023.

Our medication arrived jammed securely between two large non-sweat ice packs, wrapped tightly in protective foam, inside a pill bottle inside a ziplock. Nothing could shift and nothing was damp.

The $149 price is for the Starter tier, which is tirzepatide mixed with vitamin B6 rather than tirzepatide alone. That is not a straight comparison with the other prices here.

What we likedWhat we did not
Interactive pricing tool by pharmacy and by dose, before signupSupport was hard to reach as a non-patient; we got looped
Five named pharmacies, nearly all with clean recordsStarter tier is tirzepatide plus B6, not tirzepatide alone
Excellent packaging, medication completely immobilized$25 fee for appointments canceled inside 24 hours
Clear HIPAA policy on its own pageOne Instagram ad quoted a bundle rate as a monthly price

4. Willow

Willow compounded tirzepatide

Three pharmacies with full addresses. Score 74. From $299 a month.

Willow publishes its pharmacies with street addresses: Red Rock Home Pharmacy in Springville, Utah; Epiq Pharmacy in Richardson, Texas; and The Pharmacy Hub in Miami. Red Rock Home holds an active Class A license with two paid citations. Epiq Scripts has an active license with no disciplinary orders, though inspection reports are on file.

The packaging was among the best we saw. The medication came vacuum-sealed in its own ziplock, stapled to a larger ziplock holding the supplies, with non-sweat ice packs in thick insulation. Everything was secure and dry. Willow also included photo injection instructions and a fridge magnet for tracking your dose dates.

Tracking was the weak point. It came only from the pharmacy, not from Willow, and we got the delivery email two days after the package actually arrived.

Pricing is genuinely clear, listed on the main site with no intake required. The only extra cost is lab work if you need it.

What we likedWhat we did not

Best-protected package we opened$299 a month is the highest starting price here
Pricing clear on the main site, no intake neededRefund only if you cancel before the prescription reaches the pharmacy

5. OnlineSemaglutide.org

OnlineSemaglutide.org compounded tirzepatide

Four pharmacies listed in About Us. Score 61. From $241 a month.

The pharmacies are Belmar, Strive, Epiq Scripts, and Casa Pharma Rx, listed openly in the About Us page. Belmar holds active Florida compounding licenses with no discipline or complaints. Strive has resolved actions across its locations. The site also names its prescribers, Beluga Health, with individual doctors and NPI numbers, which is rare.

This site is unusually straight about what it sells. It states plainly that compounded semaglutide and tirzepatide are not FDA-approved and are not the same as the brand-name drugs.

The box was oversized with a lot of empty space, though the medication was bubble-wrapped and the contents were sealed against the one sweaty ice pack. There was a thank-you card with a clear visual injection guide.

Canceling was the worst experience of the nine. We had to explain why we were leaving, which felt uncomfortable, then wait through slow replies. We emailed on January 25, got a cancellation confirmation on January 26, then a contradicting email on January 29.

What we likedWhat we did not
Names four pharmacies and its prescribing doctors with NPI numbersCanceling required justifying ourselves and took four days
States openly that compounded drugs are not FDA-approvedLowest score on this page at 61
One price at every dose, no increases as you titrateThe $241 rate needs a six-month commitment
Clear visual injection instructions includedAdvertised a $150 bundle rate without saying $900 was due upfront

6. Shed

Shed compounded tirzepatide

Six pharmacies, one probationary certification. Score 87. From $239 a month.

Shed lists more pharmacies than anyone: Strive, Boothwyn, WP Pharma Labs, Red Rock, Promise, and Everwell. Strive’s Arizona location has a disciplinary action tied to semaglutide compounding and recordkeeping, settled with a $1,000 penalty under a 2024 consent agreement. Boothwyn also appears on Eden’s list, and Boothwyn is the pharmacy with the active probation described further down this page.

One thing to know: Shed holds probationary LegitScript certification, not full certification. LegitScript grants that status when a business currently appears to meet its standards but has previously fallen short.

The package itself was good. Medication in a bubble wrap envelope, everything in a foam box, materials sealed in a thick ziplock.

The contract is where we would slow down. Shed requires a two-month minimum. You are not refunded if you cancel inside that window, or after the third prescription ships. That condition appears in very small text at checkout, and the main medication page shows only a starting price.

What we likedWhat we did not
Lists six pharmacy partners openly in its help centerProbationary LegitScript certification, not full
Price does not change with doseTwo-month minimum with no refunds, in small print at checkout
Well-organized, secure packagingThe $239 rate requires a twelve-month commitment
Support answered directly across every channelFull pricing only appears after intake

7. MEDVi

MEDVi compounded tirzepatide

One pharmacy name, after a long wait. Score 64. $199 a month.

Getting the pharmacy name out of MEDVi took work. We asked, were told they use “multiple USA certified pharmacies,” were asked whether we had an account, and were then put on hold for a long time with no answer and no callback. An email eventually gave us one name: Link Compound Pharmacy, the pharmacy that shipped our order. It has no disciplinary actions. We could not check the others because we were never told who they are.

To its credit, MEDVi names its prescribers publicly, CareGLP Affiliated P.C.s and OpenLoop Health, and its site describes meeting with pharmacies regularly to review testing reports. That is a process rather than a published result, but nobody else describes doing even that.

The packaging was mixed. Non-sweat ice pack in an insulated foam box, but the medication was loose and upside down in a small ziplock inside a larger ziplock that was not sealed, and two alcohol wipes were damp from the ice pack.

The advertising is the real problem. We scored MEDVi zero on advertised pricing accuracy. The site quotes “starting at” prices and the ad copy shows only the first-month rate without mentioning the increase on refills. We also saw an ad asking “Why pay $1,000+ a month for Ozempic or $1,100 for Wegovy?” while MEDVi was selling Ozempic at $1,999.

What we likedWhat we did not
$199 a month, month to month, no commitmentWould only name one pharmacy, after a long hold and no callback
Names its prescribing groups publiclyAdvertising scored zero for accuracy
Describes reviewing its pharmacies’ testing reportsOuter ziplock unsealed, wipes damp on arrival
Human, responsive chat supportReal ongoing price buried in tiny FAQ text

8. Henry Meds

Henry Meds compounded tirzepatide

One pharmacy, with a federal case in its history. Score 74. GLP-1 from $179 a month.

Henry Meds gave us one pharmacy name when we asked: Tailor Made Pharmacy, which filled our order. Its record is the most serious we found. Tailor Made Compounding LLC in Nicholasville, Kentucky had its permit expire in March 2021, and the Kentucky Board of Pharmacy entered an agreed order in June 2022 after the pharmacy entered a federal plea agreement for unlawful distribution.

That history sits alongside the best unboxing experience of the nine. The vial came in a pill bottle steadied with three cotton balls, which we had not seen before and thought was genuinely clever, inside a large ziplock wrapped in thick foam with two ice packs. There were more spare sharps and alcohol wipes than anyone else provided, full temperature and injection guidance, and a supply of ondansetron for nausea at no extra charge. Most companies bill that separately.

Henry Meds does not publish a tirzepatide price. The site says GLP-1 medication starts at $179 a month, and you find out your actual cost by completing intake.

What we likedWhat we did not
Best packaging and supplies of the nine, including free nausea medicationIts pharmacy has a federal plea agreement in its history
Canceled by email, quickly and without questionsNo tirzepatide price published anywhere before intake
Cotton-ball vial packing kept the medication completely stillTracking froze on “shipped” and never updated
Names its prescriber, Colchis Medical Services PCBundle pricing appears before you know what you are buying

9. Eden

Eden compounded tirzepatide

Its pharmacy is on active probation. Score 76. $199 a month plus a $99 monthly membership.

Eden lists its pharmacies: Boothwyn, Precision, GoGoMeds, Enovex, and AbsolutePharmacy. Boothwyn Pharmacy in Kennett Square, Pennsylvania holds an active license that is currently on probation, with multiple disciplinary actions on record including several fines, with probation effective in 2025. That is the only open, unresolved pharmacy problem we found on this page.

We also could not locate a HIPAA policy anywhere in Eden’s terms or privacy policy. Every other company here has one.

The shipping was good. Two non-sweat ice packs, medication in a ziplock and a pill bottle wrapped in thick foam, supplies dry, tracking in email and portal with two-day delivery. Canceling took one button in the patient portal, the easiest of the nine.

Then there is the price. The $199 you see is medication only. Eden also requires a membership at $39 the first month and $99 every month after, which adds about $1,089 a year and brings the real total to roughly $3,516. That is the most expensive outcome on this page, from one of the lowest advertised prices.

What we likedWhat we did not
Canceling took one button in the portalBoothwyn Pharmacy is on active probation, effective 2025
Good cold-chain packaging, everything dry on arrivalNo HIPAA policy we could find anywhere
Support replied politely within 24 hours$99 monthly membership is not in the advertised price
Advertised price matched checkout exactlyAbout $3,516 a year, the most expensive here

What we still cannot tell you

A page built on checking things should be clear about what we did not check.

We did not lab-test the medication. Not one of these nine companies publishes independent results showing the tirzepatide is the strength on the label, or that it is sterile. MEDVi comes closest by saying it reviews its pharmacies’ testing reports, but a description of a process is not a result you can read. If you are looking for the company that proves its product is what it claims, it is not on this page, because as far as we can tell it does not exist yet in this market.

We also cannot promise your box comes from the same pharmacy ours did. Most of these companies use several, and route by state and stock. Our findings tell you which pharmacies a company works with and what those pharmacies’ records look like. They cannot tell you which one will fill your specific order.

And a disciplinary action is not proof of a bad product. A $750 citation from 2023 is not the same as an active probation in 2025, which is why we described what we found rather than reducing it to a pass or fail.

What we would tell a friend

If pharmacy safety is what worries you, Peak Wellness is where the evidence points. It is the only company here with no disciplinary actions on any of its pharmacies, and it publishes the addresses and the prescriber. It costs more than most of this list, so this is a trade.

If you want to know exactly where your medication comes from before you pay, GobyMeds shows you the pharmacy name during checkout. It is also the cheapest over a year, as long as you understand the price climbs about 50% past 9 mg.

If you want the clearest pricing in the market, Pomegranate has an interactive tool showing cost by pharmacy and by dose, which nobody else offers.

Two we would think hard about. Eden relies on a pharmacy that is currently on probation and has no HIPAA policy we could find. Amble, which appears in our wider testing, uses a pharmacy that is permanently closed with an expired license.

And before choosing anyone on the advertised price, check for a membership fee. Eden advertises $199 and costs about $3,516 a year once its $99 monthly fee is counted.

FAQ

Which pharmacies actually make tirzepatide for these companies?

We confirmed the pharmacies for all nine. Peak Wellness uses Epiq Scripts, Southend, AbsoluteRx, and Emerald. Pomegranate uses BPI Labs, Empower, Hallandale, RedRock, and OptioRx. GobyMeds uses CasaPharmaRx, SevenCells, VitalRx, and BPI Labs. Willow uses Red Rock Home, Epiq Scripts, and The Pharmacy Hub. OnlineSemaglutide.org uses Belmar, Strive, Epiq Scripts, and Casa Pharma Rx. Shed lists six. Eden lists five. MEDVi and Henry Meds each gave us one name when we asked.

Do any of these pharmacies have problems on record?

Yes, three worth knowing about. Boothwyn Pharmacy, used by Eden and listed by Shed, is on active probation as of 2025 with multiple fines. Tailor Made Pharmacy, which filled our Henry Meds order, entered a federal plea agreement for unlawful distribution and its permit expired in 2021. Align Pharmacy, connected to Amble, is permanently closed with an expired license. Peak Wellness, Enhance.MD, and Mochi came back with no disciplinary actions.

Is a 503B pharmacy safer than a 503A?

A 503B facility registers with the FDA, gets FDA inspections, and follows federal manufacturing rules, so oversight is heavier than a state-licensed 503A. Neither makes an FDA-approved drug when compounding tirzepatide. The practical difference is that a 503B leaves a federal inspection record you can look up.

Will my price go up as my dose goes up?

At some companies, yes. GobyMeds goes from a $399 starter bundle to $499 for plans up to 9 mg a week, then $599 at 11 mg or higher. Peak Wellness, OnlineSemaglutide.org, and Enhance.MD all state publicly that the price is the same at every dose. Several others do not publish dose pricing at all, so you find out while you are already taking it.

Did you actually buy from all of these companies?

Yes. We paid for our own orders with our own cards, received and photographed the packaging, contacted support through every channel offered, and cancelled each account. What we did not do is send the medication for independent lab testing, so we cannot speak to the strength or sterility of what arrived.

Is compounded tirzepatide legal?

Yes. Licensed doctors can prescribe it and licensed pharmacies can make it for an individual patient. But compounded tirzepatide is not FDA-approved, the FDA does not review it for safety, quality, or effectiveness, and it is not the same product as Mounjaro or Zepbound.

How current is this?

Every price and pharmacy claim was re-checked against the company’s own website on July 31, 2026. Pharmacy board records were checked during our testing. Prices in this market move often, so confirm the current price before you sign up.
