SUBJECT: Income Eligibility Applications
   

All Child and Adult Care Food Program sponsors must have on file an Income Eligibility Application for each enrolled participant classified as Free or Reduced. Centers must have CAC-11 or CAC11-A and Sponsoring Organizations of Day Care Homes must have CAC - 11B, CAC - 11B supplement and CAC -11C forms on file for family day care homes tiered by income. The completed, signed and dated application must be current as of July 1, 2000.

Included in this mailing is a copy of the income eligibility applications for centers (CAC 11 or CAC-11A) for fiscal year 2000-2001. These applications should be used for the period of July 1, 2000 through June 30, 2001. Sponsoring Organizations of Day Care Homes will receive the CAC 11B, CAC 11B supplement, and CAC 11C at the sponsoring organization meeting which will be held on June 21, 2000.
