All Child and Adult Care Food Program sponsors must have on file an Income Eligibility Application for each enrolled participant classified as Free or Reduced. Centers must have CAC-11 or CAC11-A. The CAC-11 is also available in Spanish. The completed, signed and dated application must be current as of July 1, 2001. Enclosed is a copy of the income eligibility applications for centers for fiscal year 2001-2002. These applications should be used for the period of July 1, 2001 through June 30, 2002.
