SUBJECT: Income Eligibility Applications
   

All Child and Adult Care Food Program sponsors must have on file an Income Eligibility Application for each enrolled participant classified as Free or Reduced. Centers must have CAC-11 or CAC11-A.  The CAC-11 is also available in Spanish. The completed, signed and dated application must be current as of July 1, 2001.

Enclosed is a copy of the income eligibility applications for centers for fiscal year 2001-2002. These applications should be used for the period of July 1, 2001 through June 30, 2002.
