Occasionally, we receive complaints on institutions and facilities participating in the CACFP. The State Agency (SA) will investigate complaints we receive on institutions. However, it is the Sponsoring Organization's responsibility to investigate complaints on the facilities (occasionally the SA will investigate these complaints.) Enclosed is a form for you to use. Instructions are on the back of the form. If the filed complaint is a Civil Rights complaint, please complete the portion that deals with civil rights and forward to Nutrition Services Branch, Special Nutrition Program Unit. USDA will investigate civil rights complaints.
